The manifestation of the economic risk has repercussions in: obtaining profit from operating activities, the maximization of the enterpriseā€™s value and the increase of the market share. The operational risk can be detected by the method of forecast analysis ā€œcost-volume-profitā€ or via coefficient of lever exploitation. This paper aims, on the basis of the information of a commerce company, to reveal the influence of risk in assessing the enterprise, as well as the possibilities to maximize its value and the increase in economic and financial performance in a competitive environment.     Analysis of building energy upgrade technologies for implementing the dual energy efficiency and demand response scheme for non-residential buildings

    The continuous growth of renewable energy and the transition to a more de-centralised electricity generation adds significant complexity to balance power supply and demand in the grid. These imbalances are partially compensated by demand response programs, which represent a new business opportunity in the building sector, especially for ESCOs. Including demand response to their traditional energy efficiency-based business model adds an additional revenue stream that could potentially shorten payback periods of energy renovation projects.     Radio-induced brain lesions

    Introduction: Radiotherapy, an important tool in multimodal oncologic treatment, can cause radio-induced brain lesion development after a long period of time following irradiation.Material and method: We report 4 cases with radio-induced brain lesions, admitted into the Fourth Department of Neurosurgery, Emergency Clinical Hospital Bagdasar-Arseni, during a 4 years period of time.Results: Two patients had meningiomas and two had unruptured cavernomas. Other side effects of radiotherapy, such as diffuse brain atrophy, leukoencephalopathy, optic atrophy, panhypopituitarism were also noted. The two patients with large meningiomas underwent surgery, with good outcome. Observation was the choice for the two asymptomatic cavernomas. Panhypopituitarism needed synthetic hormonal replacement therapy.Conclusions: Radiotherapy can cause long-term complications and can induce development of new brain lesions into previous radiation area. Meningiomas and cavernomas can be radio-induced brain lesions. Meningiomas can grow to large size, requiring surgery. Unruptured asymptomatic cavernomas can be left in place and patients are followed clinical and with serial imaging. Other findings after radiotherapy are diffuse brain atrophy, leukoencephalopathy, optic atrophy and panhypopituitarism

    Design of knowledge-based systems for automated deployment of building management services

    Despite its high potential, the building's sector lags behind in reducing its energy demand. Tremendous savings can be achieved by deploying building management services during operation, however, the manual deployment of these services needs to be undertaken by experts and it is a tedious, time and cost consuming task. It requires detailed expert knowledge to match the diverse requirements of services with the present constellation of envelope, equipment and automation system in a target building. To enable the widespread deployment of these services, this knowledge-intensive task needs to be automated. Knowledge-based methods solve this task, however, their widespread adoption is hampered and solutions proposed in the past do not stick to basic principles of state of the art knowledge engineering methods. To fill this gap we present a novel methodological approach for the design of knowledge-based systems for the automated deployment of building management services. The approach covers the essential steps and best practices: (1) representation of terminological knowledge of a building and its systems based on well-established knowledge engineering methods; (2) representation and capturing of assertional knowledge on a real building portfolio based on open standards; and (3) use of the acquired knowledge for the automated deployment of building management services to increase the energy efficiency of buildings during operation. We validate the methodological approach by deploying it in a real-world large-scale European pilot on a diverse portfolio of buildings and a novel set of building management services.     Clinical aspects, management and outcome of brain arteriovenous malformations: Results with microsurgery first policy

    We performed a retrospective study, including patients operated for brain AVMs between 1999 and 2014, in the Clinic of Neurosurgery, Emergency Clinical Hospital Bagdasar-Arseni, Bucharest. 277 patients underwent surgery for brain AVMs. Mean age was 29.82 years. 195 patients (70.40%) presented with hemorrhage and 86 cases (31.05%) were admitted with seizures. We performed total resection of AVMs in 228 cases (82.31%) and subtotal resection in 49 cases (17.69%). Regarding patients with residual nidus, 16 of them underwent second surgery, 27 stereotactic radiosurgery Gamma Knife, 3 embolization and 3 refused further treatment. Modified Rankin Scale (mRS) improved following surgery (Z = -9.248, p = 0.000). Early complications (0-30 days) were encountered in 84 patients (30.32%). We found the following risk factors for postoperative complications occurrence: motor deficit (p = 0.006), co-morbidities (p = 0.023), higher mRS (p = 0.005), lower Karnofsky score (p = 0.003), lower GCS (p = 0.016), profound nidus (p = 0.001), eloquent aria (p = 0.000), large nidus (p = 0.000), multiple arterial territory (p = 0.000), deep feeding arteries (p = 0.000), higher number of feeding arteries (p = 0.000), deep venous drainage (p = 0.000), multiple draining veins (p = 0.000), higher Spetzler-Martin grade (p = 0.006), high flow (p = 0.000), vascular steel (p = 0.000), associated aneurysms (p = 0.010) and decompressive craniectomy (p = 0.019). Mortality was 6.1%. Microsurgery is the treatment of choice for brain AVMs. Surgical results are excellent, with low morbidity and mortality. Patients with poor surgical results belonged to the group admitted with severe altered general state, state of consciousness, massive hematomas and acute brainstem dysfunction. If part of the nidus cannot be safely surgical resected, stereotactic radiosurgery can provide definitive cure of the lesion.Key words: arteriovenous malformations, microsurgery, postoperative complications

    Periodontal Medicine: Impact of Periodontal Status on Pregnancy Outcomes and Carcinogenesis

    Periodontal medicine is a broad term commonly used to define the relationship between periodontitis and systemic health. Periodontitis is a highly prevalent, chronic multifactorial infectious disease, induced by the dysbiotic biofilm that triggers a persistent systemic inflammation and recurrent bacteremia. There is a growing body of scientific evidence that suggests the potential implication of periodontitis in the causation and progression of various systemic disease and conditions, such as diabetes, cardiovascular disease, pulmonary disease, adverse pregnancy outcomes and cancer. Some studies consider periodontitis as an independent risk factor for preterm birth, growth restriction, low birth-weight and pre-eclampsia. However not all studies support the association. Despite sparse scientific data, some studies indicate that individuals with periodontitis are at increased risk for cancer development, due to the increased inflammatory burden sustained by the presence of periodontal pathogens. This chapter emphasis the relationship between periodontitis and adverse pregnancy outcomes and the underlying mechanisms that link peridontitis to oral carcinogenesis

    Periodontitis and Heart Disease: Current Perspectives on the Associative Relationships and Preventive Impact

    Due to the important advancement and the accumulation of new evidence on the periodontitis-cardiovascular disease (CVD) relationship as well as the major medical, economic and social burden caused by both diseases this chapter aims to review existing epidemiological and pathogenetic links related to this topic. Also, this chapter aims to highlight the impact of the periodontitis-CVD relationships on clinical practice and on the preventive approaches targeting to decrease the impact of periodontitis on CVD. Periodontitis is an infectious disease eliciting local and general inflammation, which leads to periodontal destruction and systemic involvement. Several pathways could explain the link between periodontitis and CVD such as bacteraemia, chronic persistent systemic inflammation and oxidative stress. The first step in the treatment of periodontitis addresses the elimination of microbial components, which lead to a decrease in local and systemic inflammation. Periodontal therapy seems to positively impact CVD. Specialists should inform patients with CVD on the negative impact of periodontitis on their systemic status and refer patients to the periodontist for an extensive examination as routine management of CVD. Some possible risks of periodontal therapy should be considered in patients undergoing antithrombotic medication
